The MRI exam allows physicians to detect sites of deep endometriosis by analyzing the uterus, ovaries, bladder, bowel and surrounding spaces in the pelvis. This can help with surgical planning and removal of tissue.
Laparoscopy was significantly more accurate than MRI in diagnosing idiopathic CPP (p < 0.0001), superficial peritoneal endometriosis (p < 0.0001), deep-infiltrating endometriosis (p < 0.0001) and endometrioma of the ovary (p = 0.02) as the cause of pelvic pain.
We showed that ultrasound is an accurate method to detect moderate or severe pelvic endometriosis comparable to surgery and magnetic resonance imaging (MRI). We are not only able to detect it, but we can also assess the extent of the disease and the spread to other organs such as bowel and urinary tract.
The appearances of endometriomas can be quite variable. The classical example is an avascular unilocular cyst containing low-level, homogeneous "ground-glass" like internal echoes, as a result of the hemorrhagic debris. This appearance occurs in 50% of cases 7.
Endometriosis cysts are better diagnosed with TVS, whereas torus, uterosacral ligaments, intestine, and bladder endometriosis lesions are best diagnosed using MRI. When it comes to detecting intestine or rectal nodules, as well as rectovaginal septum nodules, MRI should be the imaging tool of choice.
An experienced gynecologist may suspect endometriosis based on a woman's symptoms and the findings during a pelvic exam. Currently, the only way to diagnose endometriosis is through laparoscopy – a minor minimally invasive surgical procedure that is done under general anesthesia (while the patient is asleep).
The specificity of MRI in the detection of pelvic adhesions is consistently over 90% in all locations with the exception of the posterior cul-de-sac. Furthermore, the results are consistent between two independent radiologists. These findings demonstrate the utility of preoperative MRI in evaluating adhesive disease.
There is no lab test, procedure or imaging that can be done to diagnose endometriosis without surgery. However, imaging studies can be useful to look for signs of endometriosis. Common diagnostic imaging exams include: Ultrasound: This procedure uses sound waves to view organs.
For the first time in three decades, women living with endometriosis will now have access to a new treatment on the PBS. Dienogest (Visanne) will be listed for the first time to treat endometriosis. Visanne, a daily tablet, works to shrink and suppress the growth of abnormal tissue.
Endometriosis is particularly difficult to diagnose because it can have many different clinical presentations. In addition, some women may not even experience pain (asymptomatic). It's somewhat of a paradox with endometriosis that those with the largest amount of endometrial implants have the least amount of pain.
